- Air India crash report dispute : The report highlighted an advisory issued by the US Federal Aviation Administration (FAA) in 2018 regarding the potential disengagement of the fuel control switch locking mechanism in similar Boeing models. Air India had not carried out the recommended inspections because they were non-mandatory. This raised questions about whether a mechanical fault in the fuel switches could have caused the engines to shut down. The FAA and Boeing, however, maintained that the fuel switch locks were safe. The Indian Pilots’ Guild has disputed the preliminary findings of the Air India crash investigation, alleging bias.
- Â The pilot associations also criticized the investigation for a perceived lack of transparency. They pointed to the selective release of information, the late-night timing of the report’s release, and the absence of certain details like the full cockpit voice recording and cockpit conversation timestamps. This secrecy, some argued, eroded public trust and fuelled suspicion of a bias towards pilot error.
- Focus on Fuel Switches & Pilot Action:Â The preliminary report from India’s Aircraft Accident Investigation Bureau (AAIB) found that the engine fuel switches on the Boeing 787-8 moved from “RUN” to “CUTOFF” seconds after takeoff, leading to engine failure. The cockpit voice recording captured one pilot asking the other why the fuel was cut off, and the other pilot responding that they didn’t do so.

Monsoon floods continue to impact Himachal Pradesh, with fatalities and road closures, particularly in the Mandi district.
